Delisted Provider with equitable life: Please check with your insurance before you visit this eye care. When I visited them yesterday and provided my insurance details, the admin said for equitable, I would have to pay them and file online to get claim. They are a delisted provider with equitable insurance. I submitted my claim and got rejected. They provided reasonable service though and were friendly.
